To request an initial appointment please follow these 3 steps:

1.  Select the day and time below that works best for you and provide your name / e-mail / phone number. 

2. We’ll immediately send you an e-mail confirming that we’ve received your request, along with a link to download the forms for your visit.

3. We’ll call you (within 24 hours or on the following Monday if contacting us Thursday-Saturday) to notify you of the next available appointment date that falls on the day/time that you requested.
